Case Management

TMSC brings together services, agencies, resources, and/or people within a planned framework of action. Each plan addresses the achievement of jointly established goals for each family. The action plan involves liaison activities and collateral contacts.

TMSC staff coordinates the multiple service plan that includes:

1. Problem identification,
2. Assessment,
3. Planning,
4. Linkage,
5. Monitoring,
6. Advocacy, and
7. Crisis intervention.

Referrals for outside assistance are provided for problems related, but not limited, to:

1. Substance abuse,
2. Educational and/or vocational training,
3. Job placement,
4. Parental and life skills,
5. Housing.

TMSC successfully helps clients prepare short and long-term plans focused on attaining, securing, and maintaining self-sufficiency and independence.
